Southwest Washington & the Gorge: what shapes the choice here
Southwest Washington is three markets. Vancouver and Clark County have a deep Advantage menu shaped by Portland: PeaceHealth Southwest, Legacy Salmon Creek and the Vancouver Clinic on this side, OHSU and Kaiser Permanente Northwest across the bridges, and a VA campus in Vancouver that belongs to the Portland system. Olympia and Thurston County have Providence St. Peter and MultiCare Capital, plus thousands of state retirees whose PEBB coverage has its own Medicare rules.
West and south of both, the menu thins fast. Grays Harbor, Pacific, Wahkiakum, Skamania and Klickitat counties each lean on a small hospital or hospital district, and a specialist means Olympia, Vancouver or Portland. Out there, an Advantage plan with a network that stops at the county line can be a poor bargain, and Original Medicare with a Medigap policy is often the practical answer.
What to watch: Clark County’s Medicare market is really a Portland market: Kaiser Permanente Northwest, Legacy and PeaceHealth all straddle the river, and many Advantage networks are built for both states. Thurston County is Providence and MultiCare territory with a large state-retiree population on PEBB. The coast counties and the Gorge have thin menus and long drives.

Turning 65? Your Initial Enrollment Period is the 7 months around your birthday. Already on Medicare? The Annual Election Period is Oct 15–Dec 7, and Medicare Advantage Open Enrollment runs Jan 1–Mar 31. Already have a Medigap policy? Washington lets you change it any time of year.
