The Triangle: what shapes the choice here
The Triangle has the second-deepest Medicare menu in the state and the most institutional care map: Duke, UNC and WakeMed each run their own hospitals and clinics and sign their own Advantage contracts. A plan that includes Duke may exclude UNC Rex; a plan that includes WakeMed may exclude Duke Raleigh. Because so many Triangle retirees are on the State Health Plan’s Humana group plan, a lot of “which plan” questions here are really “should I stay on the State Health Plan” questions.
The 2026 UNC Health network exit is the biggest local change in years. If you are on a Humana, WellCare or Cigna Advantage plan and your doctors are at UNC Hospitals, UNC Rex or UNC Johnston, the October 15 to December 7 window is when to fix it. We check your providers against the plans sold at your ZIP code before you enroll.
Local plans to know: Experience Health is a Medicare Advantage HMO built by Blue Cross NC and Duke Health with Duke and WakeMed Key Community Care providers; Alignment Health sells in Wake, Orange, Johnston and Chatham counties for 2026. And effective January 1, 2026, UNC Health is out of network with Humana, WellCare and HCSC (formerly Cigna) Advantage plans — State Health Plan retirees on Humana’s group plan are the exception.

When you can enroll
Turning 65? Your Initial Enrollment Period is the 7 months around your birthday. Already on Medicare? The Annual Election Period is Oct 15–Dec 7, and Medicare Advantage Open Enrollment runs Jan 1–Mar 31. Moving into North Carolina, or a plan leaving your county, opens a Special Enrollment Period.
