Charlotte Metro: what shapes the choice here

More than a quarter of a million people on Medicare live in the Charlotte metro, and the plan menu shows it: dozens of Medicare Advantage plans in Mecklenburg County, every Medigap carrier licensed in North Carolina, and the full Part D lineup. The trade-off is that care here is organized into large systems that contract separately. The first question is never the premium; it is whether your doctors belong to Atrium, Novant, CaroMont or, on Lake Norman, Duke.

Outside Mecklenburg the menu thins a little but the same rule holds. Cabarrus County leans on Atrium Health Cabarrus, Gaston County on CaroMont, Iredell on Duke Health Lake Norman and Iredell Memorial, Rowan on Novant Health Rowan and the Salisbury VA. What to know locally: Charlotte is a two-system town. Atrium Health and Novant Health each run their own hospitals and clinics and each contract on their own terms, so a $0-premium plan that includes Atrium may exclude Novant or the reverse. Duke Health took over the Mooresville hospital in April 2025, which changes the Lake Norman referral map.

When you can enroll

Turning 65? Your Initial Enrollment Period is the 7 months around your birthday. Already on Medicare? The Annual Election Period is Oct 15–Dec 7, and Medicare Advantage Open Enrollment runs Jan 1–Mar 31. Moving into North Carolina, or a plan leaving your county, opens a Special Enrollment Period.
