Cary and the western Wake towns have WakeMed Cary as the local hospital and Duke, UNC and WakeMed Raleigh all within twenty minutes — which makes the network question a three-way one.
What to know before you compare plans in Wake County
Wake County offers dozens of Medicare Advantage plans, every Medigap carrier in North Carolina and the full Part D lineup. WakeMed Cary Hospital is the only full-service hospital in town and holds a Level 1 geriatric emergency department accreditation; Duke and UNC Rex are close enough that most Cary residents use more than one system, which an HMO network may or may not allow. For 2026 UNC Health is out of network with Humana, WellCare and HCSC (Cigna) Advantage plans.
Cary also draws retirees from the Northeast and from India and China who are new to Medicare entirely, and a relocation or first-enrollment window comes with them. We compare what is sold at your ZIP code and confirm every doctor you use.

When you can enroll
Turning 65? Your Initial Enrollment Period is the 7 months around your birthday. Already on Medicare? The Annual Election Period is Oct 15–Dec 7, and Medicare Advantage Open Enrollment runs Jan 1–Mar 31. Moving into North Carolina, or a plan leaving your county, opens a Special Enrollment Period.
Your options
Which kind of plan fits you in Cary?
Medicare Advantage
All-in-one Part C plans, often $0 premium, that use a local network — so which hospital system you use, your county and your prescriptions matter.
Medigap pairs with Original Medicare and lets you see any provider nationwide that accepts Medicare — Duke, UNC, Atrium, Mayo, or the clinic in your county seat.
Wake County typically offers dozens of Medicare Advantage plans, every Medigap carrier in North Carolina, and the full Part D lineup, set by ZIP code and changing each plan year.
Can I use WakeMed, Duke and UNC with one plan?
With Original Medicare and a Medigap policy, yes. With an Advantage plan only if all three are in that plan’s network, which is rare; some PPOs cover out-of-network care at higher cost. We check each before you enroll.
How much does your help cost?
Nothing. Independent agents are paid by the carriers when you enroll, so comparing plans and reviewing your coverage each year is free to you.
