Hilo is East Hawaii’s hub: Hilo Benioff Medical Center, the Hilo VA clinic, the state veterans home — and a 50-minute flight to Oahu for the subspecialists the island does not have.
What to know before you compare plans in Hawaii County
Hawaii County offers Medicare Advantage plans from Kaiser Permanente (most of the county), HMSA’s neighbor island Akamai Advantage plans and some national carriers, every Medigap company licensed in Hawaii and the full Part D lineup. Hilo Benioff Medical Center, renamed in 2024 as an $80 million expansion began, is the largest hospital in the Hawaii Health Systems Corporation and the anchor for Puna, Hamakua and Kau. Patients drive in from Pahoa, Keaau, Volcano and Honokaa, so a plan’s network has to reach the whole east side.
What Hilo cannot do, Oahu does. A Medigap policy covers Queen’s or Straub without a referral or travel rule; HMSA’s neighbor island plans include interisland travel help when they refer you off-island; Kaiser refers within Kaiser. We read those rules with you before you enroll, and for veterans we check how the Hilo VA clinic fits.
Care close to home
Major care in the area includes:
Hilo Benioff Medical Center (HHSC)
Hilo VA Community Based Outpatient Clinic
Yukio Okutsu State Veterans Home
Kau Hospital (HHSC, Pahala)
Hale Hoola Hamakua (HHSC, Honokaa)
Plan networks differ — we check that your doctors and hospital are covered before you enroll.

Your options
Which kind of plan fits you in Hilo?
Medicare Advantage
All-in-one Part C plans, often $0 premium, that use a local network — in Hawaii that usually means choosing between Kaiser, HMSA and a few national carriers, island by island.
Medigap pairs with Original Medicare and lets you see any provider that accepts Medicare — Queen’s, Straub, Hilo Benioff, or a specialist on the mainland.
Hawaii County typically offers Medicare Advantage plans from Kaiser Permanente (most of the county), HMSA’s neighbor island plans and some national carriers, every Medigap company licensed in Hawaii, and the full Part D lineup, changing each plan year.
I have to fly to Oahu for a specialist. Which plan covers that?
Original Medicare with a Medigap policy covers any provider that accepts Medicare on any island or the mainland, with no referral and no travel rule. HMSA’s neighbor island Akamai Advantage plans include help with interisland travel when HMSA refers you to a participating specialist on another island; Kaiser refers within its own system. We read the plan’s travel rules with you before you enroll.
How much does your help cost?
Nothing. Independent agents are paid by the carriers when you enroll, so comparing plans and reviewing your coverage each year is free to you.
