The Midlands: what shapes the choice here

The Midlands have the second-deepest Medicare menu in South Carolina and a care map that runs along the river: Prisma Health Richland and Baptist on the Columbia side, Lexington Medical Center on the West Columbia side, and MUSC Health’s downtown Columbia hospitals, each signing its own Advantage contracts. Which system your doctors belong to decides the shortlist before premiums do.

The region also holds Fort Jackson and the Army retiree community around it, plus state and school-district retirees on PEBA coverage, and the Dorn VA, which serves veterans from the whole middle of the state. Orangeburg and Newberry rely on one hospital apiece, and in the rural counties between them a Medigap policy’s freedom to use any of the Columbia systems is often the practical answer.

What shapes the choice here: Columbia has three systems that contract on their own terms — Prisma, Lexington Medical Center and MUSC Health — and one of the state’s largest military-retiree populations, for whom TRICARE For Life changes what “best plan” means. Fairfield, Saluda and Calhoun counties have thinner menus and longer drives.

When you can enroll

Turning 65? Your Initial Enrollment Period is the 7 months around your birthday. Already on Medicare? The Annual Election Period is Oct 15–Dec 7, and Medicare Advantage Open Enrollment runs Jan 1–Mar 31. Moving to South Carolina, losing a plan, or a FEMA hurricane declaration each open a Special Enrollment Period.
