Medicare in the region
San Antonio is one of the most Advantage-heavy Medicare markets in America, with University Health, Methodist, Baptist and Christus each contracting on their own terms and a large military-retiree population using TRICARE For Life on top of Medicare. The plan that fits a retired master sergeant in Universal City is rarely the plan that fits a retiree in Alamo Heights.
South and west of the city, the menu thins fast. Laredo, Del Rio, Eagle Pass and Uvalde each rely on one or two hospitals, and rural South Texas was among the areas where carriers pulled plans for 2026.
Local plans to know: Bexar County has one of the highest Medicare Advantage enrollment rates of any large county in the country, and a large TRICARE For Life population that changes what “best plan” means. Webb County (Laredo) and the brush-country counties have thin menus and, for 2026, carrier withdrawals.

Turning 65? Your Initial Enrollment Period is the 7 months around your birthday. Already on Medicare? The Annual Election Period is Oct 15–Dec 7, and Medicare Advantage Open Enrollment runs Jan 1–Mar 31. A plan leaving your county opens a Special Enrollment Period.
