Why people buy it

The problem it solves

Medicare pays for home health care only under narrow conditions, and it stops well before most people expect. What it does not pay for is the help someone needs for a few weeks or months after a hip replacement, a stroke or a bad fall — the bathing, dressing and getting safely out of a chair.

That gap usually gets filled by a daughter taking unpaid leave. Short-term home health care insurance pays a daily cash benefit for that period instead, on a maximum benefit period measured in months rather than years.

How it works

Three things happen, in this order

A licensed health care practitioner certifies you need help

The trigger is cognitive impairment, or needing substantial assistance with at least two of six activities of daily living: bathing, continence, dressing, eating, toileting and transferring.

Care is provided at home

Skilled nursing, therapy, a home health aide — covered services vary by plan, and the daily benefit is capped at a combined maximum.

The benefit is paid to you in cash

Not to an agency. Some plans also pay a lump sum to the family member doing the caring, which is unusual and worth asking about.

Who this suits, and who it does not

The second list matters more than the first. An agent who only writes the first one is selling, not advising.

Worth considering if…

  • You want protection for the recovery period after surgery or a fall, not for a multi-year stay in a facility.
  • Traditional long-term care insurance was declined, or priced beyond what you will commit to, and doing nothing feels worse than doing something smaller.
  • There is a family member who would end up providing the care, and you would rather that came with money attached.

Probably not for you if…

  • You need genuine long-term care protection. This is not it, the policy says so, and buying this in the belief that it is would be the most expensive mistake on this page. Benefit periods here are measured in months.
  • You want nursing home or assisted living coverage. The benefit is for care at home. Facility care is a different product.
  • You already need help with two or more activities of daily living. Coverage has to be in force before the need arises, and applications ask about this directly.

Every benefit below is transcribed from GTL Short-Term Home Health Care brochure GAD15.1-23 (Rev. 5/24) 15B644. Benefits, amounts and riders vary by state; the policy issued to you governs.

  • Home health care daily benefit Included

    A daily cash benefit for covered home health care. Combined daily maximum of $150 on Plan A, $300 on Plan B and $450 on Plan C, across skilled and general nursing, physical, speech, occupational, chemotherapy, enterostomal and respiratory therapy, medical social services and a home health aide. Maximum benefit period 360 days.

    In ID the maximum benefit period is 280 days.

  • Home health care aide benefit Included

    $50, $100 or $150 a day by plan. No prior hospital stay is required.

    Counts toward the combined daily maximum.

  • Prescription drug benefit Included

    $10 for each generic and $25 for each brand-name prescription, up to $300, $600 or $900 a policy year by plan.

  • Family caregiver benefit Included

    A $3,500 lump sum once a home health care claim is approved and your family caregiver has registered and completed a caregiver assessment with TCARE. Payable to you, or assignable to your caregiver.

    Benefits assigned to an informal caregiver may have tax consequences for that person. GTL and its agents do not give legal, financial or tax advice.

  • Restoration of benefits Included

    Benefits restore after 180 consecutive days without covered services, once a licensed health care practitioner certifies you have recovered sufficiently to no longer need home or nursing care.

  • Accident and sickness hospitalization rider Optional rider

    Up to $300 a day by plan for hospital confinement of at least 24 hours, over a 3-day or 6-day benefit period, restoring after 60 days.

    ID and PA include a reduced daily benefit for the remainder of a 31-day maximum benefit period.

  • Ambulance rider Optional rider

    $200 for ground ambulance to or from a medical facility, up to four times a year, $2,500 lifetime. No hospital confinement required.

  • Return of premium rider Optional rider

    Returns premiums paid, less benefits paid, on your death.

    Not offered by GTL in Georgia, Tennessee.

  • Dental and vision rider Optional rider

    Up to $400, $800 or $1,200 a year for a licensed dentist, ophthalmologist or optometrist after the first year, including $200 toward prescription glasses or contact lenses.

    Not offered by GTL in Colorado, Georgia, Tennessee.

  • Critical accident rider Optional rider

    After an emergency room visit, a lump sum by injury: accidental death $5,000 or $10,000; hip or skull fracture $1,250 or $2,500; hip dislocation $1,000 or $2,000; knee dislocation or ligament tear $500 or $1,000; other fracture $250 or $500.

    Not offered by GTL in Colorado, Tennessee.

Questions people actually ask

Is this long-term care insurance?

No, and the policy states that plainly. It uses the same benefit trigger as long-term care insurance — cognitive impairment or needing help with two of six activities of daily living — but the maximum benefit period is measured in months rather than years, and it is not tax-qualified long-term care coverage under the Internal Revenue Code. It is designed for recovery, not for permanent decline.

How is it different from real long-term care insurance?

Duration and tax treatment, mainly. Long-term care insurance covers years of care, often in a facility as well as at home, and a tax-qualified policy carries specific advantages and protections. Short-term home health care covers a defined number of days of care at home. It is cheaper and far easier to qualify for, and it is not a substitute.

Does Medicare not already pay for home health care?

Only in limited circumstances and usually not for long. Medicare generally requires that you are homebound and need intermittent skilled care, and it does not cover custodial help — assistance with bathing, dressing and moving around — when that is the only care you need. That custodial gap is exactly what this product is built for.

What is the family caregiver benefit?

Some policies pay a lump sum to the family member providing your care once a claim is approved and they have completed a caregiver assessment. It recognizes that most home care is delivered by relatives at their own expense. Benefits assigned to an informal caregiver may have tax consequences for them, so that person should take their own advice.
